斎宮は水の尾の御時、文徳天皇の御むすめ、惟喬親王の妹。
あるとき男は狩の使として伊勢国へ下った。狩の使とは、鳥獣を狩るため諸国に派遣された使者のことで、男はその正使であった。彼はここ伊勢国で、斎宮の世話を受けることとなった。
斎宮が親に「あのかたは特別だからしっかりお世話なさい」と言われていたこともあって、男は丁重なもてなしを受けた。朝には狩の支度をして送り出してもらい、夕方戻ってくれば斎宮とひとつ屋根の下の対屋に迎え入れられた。
こうして二日目の夜。男は惑乱して斎宮に訴えた、「お逢いしたい」と。
斎宮も逢いたくないと言えば嘘になるが、なにせ人目が多く思うに任せない。
この男は正使ゆえ、斎宮の寝所の近くに床をとっていた。神に仕える女の身の上を思えば逢うことなど絶望的ですらあるが、なおも寝られない。ふと外に目をやると、真夜中の朧な月明かりのなか、童女を先に立てて人が立っている。皆が寝静まるのを待ってやってきた斎宮であった。
男は自分の寝所に女を導いた。
一刻ほどして、女は帰っていった。まだ満足に語り合っていない。男は切なさに一睡もできなかった。
翌朝、まさか男の方から後朝の文を送るわけにもいかないので、向こうから何か言ってこないかと待ち焦がれていると、空も明るくなってからただ歌のみ届けられた。
「君や来し我や行きけむ 思ほえず 夢かうつつか寝てか覚めてか
(あなたが来たのか私が行ったのか、夢か現実か寝ていたのか覚めてたのか…何もわからないの)」
男は涙がこらえきれない。
「かきくらす心の闇に惑ひにき 夢うつつとは今宵さだめよ
かきくらす こころのやみに まどひにき ゆめうつつとは こよひさだめよ
(心乱れて私もよくわからなかった…夢か現実かは今夜ご確認なさい」
そう詠み送って、男は狩りに出た。
野に出てからも男は上の空で、「今夜こそは早く皆をやすませて逢おう」などと思っている。そこへ、狩の使が来ていることを聞きつけた伊勢守が、饗応を申し出てきた。翌朝になれば伊勢を発たねばならないこともあって、今夜はいかにしても斎宮と逢いたかったが、宴となればそれもかなわない。
結局、男は斎宮に逢えぬ辛さを隠したまま、伊勢守らと酒を酌み交わすのだった。
夜もしらじら明けそめた頃、斎宮方から男のもとへ盃が差し出された。見れば、上の句のみの歌が書き添えてある。
「かち人の渡れど濡れぬえにしあれば
かちびとの わたれどぬれぬ えにしあれば
(渡っても濡れもしない浅い江のようなご縁でした)」
男は、続きを松明の燃え残りの炭で書き付けた。
「また逢坂の関は越えなん
またあふさかの せきはこえなん
(いつか必ずやお逢いできましょう」
その朝、男は尾張国へ旅立っていった。
斎宮は、惟喬親王の妹宮である―。

What Are the Benefits of Mindfulness A Practice Review of Psychotherapy-Related Research

Research suggests that mindfulness practices offer psychotherapists a way to positively affect aspects of

Mindfulness has enjoyed a tremendous surge in popularity in the
past decade, both in the popular press and in the psychotherapy
literature (Didonna, 2009a; Shapiro & Carlson, 2009). Owing
largely to the success of mindfulness-based stress reduction
(MBSR) programs and the centralrole of mindfulness in dialecti-
cal behaviortherapy, as well as acceptance and commitment
therapy, mindfulness has moved from a largely obscure Buddhist
conceptto a mainstream psychotherapy construct. Advocates of
mindfulness would have us believe that virtually every client, and
theirtherapists, would benefitfrom being mindful.In fact, mind-
fulness has been proposed as a common factorin psychotherapy
(Martin,1997). Among its theorized benefits are self-control
(Bishop et al., 2004; Masicampo & Baumeister, 2007), objectivity
(Adele & Feldman, 2004; Brown, Ryan, & Creswell, 2007; Leary
& Tate, 2007; Shapiro, Carlson, Astin, & Freedman, 2006), affect
tolerance (Fulton, 2005), enhanced flexibility (Adele & Feldman,
2004), equanimity (Morgan & Morgan, 2005),improved concen-
tration and mental clarity (Young,1997), emotionalintelligence
(Walsh & Shapiro, 2006), and the ability to relate to others and
one's self with kindness, acceptance, and compassion (Fulton,
2005; Wallace, 2001).Is mindfulness as good as advertised, how-
ever? What does the research literature have to say aboutthe
benefits of mindfulness? The purpose ofthis paperis to provide
psychotherapists with information aboutthe empirically supported
advantages of mindfulness, contextualized by effect sizes ofthese
advantages.In addition, we review research on practices that have
been found to promote mindfulness, as well as the effects on
therapists and trainees exposed to mindfulness meditation. The
paper concludes with implications for practice,reseiirch, and train-
ing. We begin by exploring the meaning ofthe term "mindful-
ness."
Definitions: Ancient and Modern
The term "mindfulness" has been used to referto a psycholog-
ical state of awareness, a practice that promotes this awareness, a
mode of processing information, and a characterologicaltrait
(Brown et al., 2007; Germer, Siege], & Fulton, 2005; Kostanski &
Hassed, 2008; Siegel, 2007b). The word mindfulness originally
comes from the Pali word sati, which means having awareness,
attention, and remembering (Bodhi, 2000). Mindfulness can sim-
ply be defined as "moment-by-moment awareness" (Germer et al.,
2005. p. 6) or as "a state of psychologicalfreedom that occurs
when attention remains quiet and limber, without attachmentto
any particular point of view" (Martin,1997. p. 291.italics included
in originaltext). Forthe purposes ofthe present paper, and forthe
sake of consistency with most ofthe research thatis reviewed
subsequently, mindfulness is defined as a moment-to-moment
awareness of one's experience withoutjudgment.In this sense,
mindfulness is viewed as a state and not a trait, and while it might
be promoted by certain practices or activities (e.g., meditation),it
is not equivalentto or synonymous with them. When slightly
different definitions of mindfulness are used in the literature thatis
reviewed,these shall be noted.
Mindfulness has similarities to other psychotherapy-related con-
structs. For example, mindfulness is similarto mentalization
(Bateman & Fonagy, 2004. 2006; Fonagy & Bateman, 2008),the
developmental process of understanding one's own and others'
behaviorin terms ofindividuals'thoughts,feelings, and desires.
Both constructs emphasize the temporary, subjective, and fluid
nature of mental states and both are thoughtto enhance affect
regulation and cognitive flexibility (Wallin, 2007). Mindfulness
differs from mentalizing in that mindfulness is both being aware of
the "reflective self engaged in mentalizing, and the practice of
fully experiencing the rising and falling of mental states with
acceptance and without attachment and judgment. (以下続く)
Implications of Attachment Theory and Reasearch for the Assessment and Treatment of Eating Disorders
In this paper, we review the research literature on attachment and eating disorders and suggest a
framework for assessing and treating attachmentfunctioning in patients with an eating disorder.
Treatment outcomes forindividuals with eating disorders tend to be moderate. Those with attachment-
associated insecurities are likely to be the leastto benefitfrom current symptom-focused therapies. We
describe the common attachment categories (secure, avoidant, anxious), and then describe domains of
attachmentfunctioning within each category: affectregulation,interpersonal style, coherence of mind,
and reflective functioning. We also note the impact of disorganized mental states related to loss or
trauma. Assessing these domains of attachmentfunctioning can guide focused interventions in the
psychotherapy of eating disorders. Case examples are presented to illustrate assessment, case formula-
tion, and group psychotherapy of eating disorders that are informed by attachmenttheory. Tailoring
treatments to improve attachmentfunctioning for patients with an eating disorder willlikely resultin
better outcomes forthose suffering from these particularly burdensome disorders.

痛みの薬
前回の記事で、鎮痛剤を濫用していそうな患者を見分けるコツを学んだと書いた。日本の医療現場でおいて役に立つかどうかは分からないが、スラム街の診療所で共に働く仲間から学んだ、それらのコツを書いてみたい。
まず当然ながら、今までクリニックに来たことのない初診の患者さんが痛みを訴えて来院したら、たとえ本人がそれまで使用していたと訴えても、麻薬系の薬は処方しない。「今のところはこれ(イブプロフェンなど)しか出せません。不満ならERに行くか、以前その薬を処方してくれた医師にかかってください」と告げる。
次に、麻薬系の鎮痛薬の愛称を使いこなす人は、疑ってかかること。パーコセットのことを「パーク」、ハイドロコドンのことを「ハイドロ」などと呼んでいる患者さんは、大いに怪しむ。「パークは2錠飲めば、効くんですよ」と言っている場合は、道ばたで「パークを買わない? 20錠買えばまとめて安くするよ」などと言っている可能性が大だ。
これら常識と思われるもの以外にも、「~の法則」と名付けられた中毒患者の見分け方がある。その中でもよく知られているのは、「金曜日の法則」だ。私たちが5時にきちんと診察を終えたがっているのを分かっていて、わざと金曜日の4時過ぎに来るというものである。特に、クリスマスの前日の午後は要注意だ!
また、前回の記事で紹介したように、触診の際、場所に関係なく一律でひどく痛がる人がいる。友人のナースプラクティショナー(NP)は、背中や肩の痛みを訴える患者さんがいると、deep tendon reflexなど色々調べた後で、もっともらしく耳たぶを引っ張ると言っていた。エビデンスのある話ではないが、本当に疾患がある場合は、ほかの診察では痛くても、耳たぶテストではほとんど痛がらない。一方、派手に叫ぶ患者さんは、仮病の疑いが強いのだという。彼女曰く「耳たぶの法則」だが、中耳炎の患者さんと出会うことのないよう願うばかりだ。
幸か不幸か、私も法則を名付ける機会に恵まれた。数年前、ある患者から「ロブスターの漁に出ていて、甲板で薬を取り出したら、船が揺れて海に全部落ちてしまった。ペインクリニック(緩和治療専門医)の予約は再来週なので、それまでの分だけでも処方してくれないか」という電話がかかってきたことがある。電話を受けたのは60代後半のベテラン看護師で、「怪しい。この近辺の沖合で船がそこまで揺れるはずはない」と妙にローカルな情報を提供してくれた。近所の薬局に手当たり次第電話をかけ、彼の処方歴を調べてみたところ、数カ所のペインクリニックやER、内科医から大量の麻薬系痛み止めを手に入れていたことが判明した。これは「ロブスターの法則」と名付けたい。
しかし、例え嘘を付いている(であろう)患者さんを見つけ出しても、処方箋を出さないことで患者を「罰して」終結できるわけではない。発見は、辛い会話のほんの始まりでしかない。痛みがうまくコントロールできていないために大量に服用していただけなのか、それとも自分で粉末にして鼻から吸引していたのか。また、余った薬を売っていたのか、ティーンエージャーの息子に脅されて手渡しているのか。
誰かが“ハイ”になるために使ったかもしれない薬を処方してしまったという空しさと、それを見抜けなかった無念さ、嘘をついていた患者への怒り。そして何よりも、そうまでして薬を手に入れていた患者への哀れみという、相反する感情で胸が一杯になる。
じっくり患者の話を聞いて、精神科や中毒患者用のプログラムの受診を懸命に勧めても、行ってくれる人は少ない。米国ではペインクリニックを受診するためには、保険の種類によっては3カ月待ちという状況にある。だから私たちは本当に痛みに苦しむ人たちを助けたくて、処方箋を書かなければいけないことがある。 今後も鎮痛剤を求める中毒患者と相対する中で、痛い経験もいくつか通過しなければいけないのだろう。 処方薬による中毒が社会を蝕んでいる限り、私たちの葛藤は続くのだと覚悟している。

-MADRSは、1979年に公表され、うつ病の臨床評価に世界中で広く使用されています。先生が、MADRSを開発しようというきっかけはどういうものでしたか?
MADRSを開発した頃の私はまだ若く、経験もあまりありませんでした。当時、同僚は項目の少ない評価尺度にはあまり関心はもっておらず、多くの項目を含む広範囲の症状評価尺度を用いた研究のほうが面白いようでした。しかし、私は、うつ病を反映する評価尺度の必要性を感じてMADRSを開発したのです。項目として、患者さんの70%にみられる症状に限定し、うつ病の症状であっても、発現頻度の非常に低い症状は変化への感度が低くなりますから、それは外しました。うつ病の主要な症状で、かつ発現頻度の高い症状を反映する尺度にしたかったのです。
-MADRSを開発される際に、どのようなところに配慮されましたか?
ひとつは、異なる文化でも理解できる症状に限定したことです。例えば、「depression」という用語は、アフリカや中東の文化では理解されないことがわかったのです。抑うつ状態はあるのですが、それを「depression」とは呼ばないのです。ですから「depression」という用語を使わず、どの文化にもある「sadness」(悲しみ)という用語にしたのです。
また、「irritability」(いらいらする、怒りっぽい)という用語は、ドイツとスウェーデンでは「anger」(怒り)と訳されます。ですから、混乱を招くような用語を使わないよう、大変気を配りました。
一方、評価尺度の感度を調べるのに、いくつかの方法を用いました。個々の項目の感度を、70%以上のうつ病患者に発現した17の症状項目全体と比較して検証し、また個々の項目およびMADRS尺度を、CGI(Clinical Global Impression)尺度と比較して検証しました。
発現頻度がもっとも高い項目が、感度がもっとも高いということではなく、たとえば、筋緊張やつまらないことを気にかけるなどの症状は、比較的多かったのですが、治療による変化には感度は低く、また食欲減退と睡眠減少の発現頻度はそれほど高くないのですが、治療による変化には敏感でした。
ちょうどその頃、DSM(Diagnostic and Statistical Manual of Mental Disorders)の委員会が開催されていたのですが、奇しくもリストの項目がMADRSとほぼ一致していたのです。MADRSは統計解析により、DSMは委員会のコンセンサスにより開発されたのですが、項目のリストは非常によく似たものだったのです。ただひとつMADRSに含まれていてDSMにはなかった項目に、tension(緊張)がありました。私は今でもこの項目はうつ病の中心的症状だと考えています。
▼臨床におけるMADRSの使用方法
-欧米では抗うつ薬を検討する多くの臨床試験で、重症度の変化を評価する尺度としてMADRSが盛んに用いられていますが、MADRSを用いてどのくらいの間隔で評価するのですか?
MADRSは特定の評価間隔を想定したものではないのですが、通常は1週ごとに前の週と比べるという方法で用いています。評価間隔が短かすぎると、項目ごとのバラツキが大きくなるという問題が出てきます。たとえば、よく眠れなかった翌日にぐっすり眠った場合など、その項目は大きく変化します。バラツキを少なくするためもあって、従来の試験では平均して7日間の状態をみているのです。こうすると、より安定した測定値が得られます。
-日本では、医師がそれぞれの患者さんに割ける時間は非常に限られているのですが、MADRSを使って患者さんを評価する際にどのくらいの時間をかけますか?
それは自分の仕事量と患者数により違ってくるでしょう。評価尺度を非常に速く使用できる医師もあれば、ずっと時間をかける医師もあります。初診時には、患者さんに、うつ病の主要症状の程度を調べる必要性を説明して、実際には10~15分かければ、正しく評価できると思います。再診の場合には、問題のある症状がどれかわかっていますので、それを評価します。うつ病を治療する際に、症状がどのように良くなっているかを把握することが重要です。
一方、寛解に入りつつある場合にはスコアは非常に低くなりますので、一つか二つの質問だけで済ませることはできません。一連の症状を尋ねて、不安や集中力、睡眠、興味などに残遺症状がないことを確認しなければなりません。
-寛解の話になりましたが、MADRSを用いた場合にカットオフ値をどうお考えですか?
健康者を対象に一連の試験を実施したことがあります。その結果、回復したうつ病と正常とを識別するカットオフ値は、MADRSの合計が12点であることが示されました。現在、大規模な試験ではこの値が用いられています。10点を提唱する人もいますが、私の経験では12点が妥当だと思います。
また、軽症うつ病は12~21点、中等症は22~30点、重症は30点以上と考えています。 重症のカットオフ値として35点とか28点を提唱する人もいるのですが、30点がもっともよく使われています。非常に重症なうつ病を診察したあとでは、次の患者さんの症状を過小評価してしまいがちで、その逆もあるでしょう。このような理由から、MADRSでは各レベルごとに症状を定義して、この問題を克服しています。
-MADRSを使用する際にとくに注意することはあるでしょうか?
私もMADRS日本語版を作成する際に、原著者としてコメントする機会をいただきました。日本語版では、日本の文化に適したものにするため細心の注意が払われており、MADRSの感度は高いようです。
MADRSを用いるときに、質問に対してあいまいな回答しか得られていない場合は、真の症状がなんであるのかを探る必要があり、最終的なスコアが正しいことを確信する必要があります。MADRSは半構造的インタビューで、1から順に質問していきますが、途中で前の項目について十分な情報が得られていないことに気付いたら、そこに戻って質問をしてください。患者さんには、このような一連の質問はこれまでなじみのないものでしょうから、すこし詳しく説明しながら質問していかなければなりません。

EFFICACY OF INTERPERSONAL THERAPY-GROUP FORMAT ADAPTED TO POST-TRAUMATIC STRESS DISORDER: AN OPEN-LABEL ADD-ON TRIAL
Background: Post-traumatic stress disorder(PTSD)is a highly prevalent
condition, yet available treatments demonstrate only modest efficacy. Exposure
therapies, considered by many to be the "gold-standard" therapy for PTSD, are
poorly tolerated by many patients and show high attrition. We evaluated
interpersonaltherapy,in a group format, adapted to PTSD (IPT-G PTSD), as
an adjunctive treatmentfor patients who failed to respond to conventional
psychopharmacologicaltreatment.
Methods: Research participants included 40
patients who soughttreatmentthrough a program on violence in the department
of psychiatry of Federal University ofSao Paulo (UNIFESP). They had received
conventional psychopharmacologicaltreatmentfor atleast12 weeks and failed
to have an adequate clinicalresponse. After signing an informed consent,
approved earlier by the UNIFESP Ethics Review Board,they received a semi-
structured diagnostic interview (SCID-I), administered by a trained mental
health worker,to confirm the presence of a PTSD diagnosis according to
DSM-IVcriteria. Otherinstruments were administered, and patients completed
out self-reportinstruments at baseline, and endpointto evaluate clinical
outcomes.
Results: Thirty-three patients completed the trial, but all had atleast
one second outcome evaluation. There were significantimprovements on all
measures, with large effect sizes.
Conclusions:IPT-G PTSD was effective not
only in decreasing symptoms of PTSD, but also in decreasing symptoms
of anxiety and depression.Itled to significantimprovements in social
adjustment and quality oflife.It was welltolerated and there were few
dropouts. Ourresults are very preliminary;they need further confirmation

Abstract: We conducted a randomized comparison of dual-focus schema
therapy with individual drug counseling as enhancements to the residential
treatment of105 substance-dependent patients with specific personality dis-
orders versus those without. Both therapies were manual-guided and delivered
for 6 months by experienced psychotherapists intensively trained and super-
vised with independentfidelity assessment. Using the Cox proportional hazards
model, we found no psychotherapy differences in retention (days in treatment).
Hierarchicallinear modeling indicated that participants with personality dis-
orders started with higher psychiatric,interpersonal, and dysphoria symptoms
and that both therapies reduced symptoms in 6 months. Contrary to predic-
tions,individual drug counseling resulted in more sustained reductions than
did dual-focus schema therapy in several symptoms for several personality
disorders. Ourfindings raised important questions aboutthe added value of
integrative or dual-focus therapies for co-occurring personality disorders and
substance dependence relative to empirically supported therapies focused more
specifically on addiction symptoms.

The co-occurrence of axis I and II disorders is very common and
associated with severe psychosocialimpairment(Crouse et al.,
2007; Mertens et al., 2003; Oldham et al., 2009). Although most
clinicians and experts believe thatthese complex patients with axis I
and II disorders need intensive orintegrative treatments,there is very
little research that supports the efficacy ofintegrative or dual-focus
models in comparison with treatments that primarily target one ofthe
disorders (Conrod and Stewart, 2005; Kienast and Foerster, 2008).
There is substantial evidence that psychotherapy and intensive psy-
chosocialtreatments are effective for personality disorders (PDs), and
many patients in more than 100 axis IItreatment outcome studies had
co-occurring axis 1 disorders (Leichsenring and Leibing, 2003; Perry
et al.,1999; Sanislow and McGlashan,1998; Verheui and Herbrink,
2007).In addition, outcomes have been evaluated for subgroups of
axis II patients in the treatment of an axis I diagnosis such as mood
(Diguer et al.,1993; Hardy et al.,1995; Shea et al.,1990), eating
(Fahy et al.,1993; Johnson et al.,1990), anxiety (Brown et al.,1995;
Stravynski et al.,1982), and substance use (Cacciola et al.,1996;
Messina et al., 2003; Woody et al.,1985) disorders. Furthermore,
many treatment outcome studies specifically targeting borderline PD
have included patients with axis I diagnoses, most commonly mood
disorders (Bateman and Fonagy,1999. 2008; Blum et al., 2008;
Clarkin et al., 2007; Davidson et al., 2006; Giesen-Bloo et al., 2006;
Hoglend,1993; Linehan et al.,1991. 2006; Ryle and Golynkina,
2000). However, very few randomized trials have been conducted on
psychotherapy models that provide an integrative or dualfocus in
managing and ameliorating the symptoms of both axis I and II dis-
orders orthatinclude a broaderrange ofPDs.
Dialectical behaviortherapy (DBT; Linehan,1993)for border-
line PD and schema therapy (Young,1994)for a broaderrange ofPDs
have both been adapted and evaluated for substance-dependent
patients in smallrandomized trials and have shown promising results
(Ball et al., 2005; Ball, 2007; Linehan et al.,1999,2002).In addition,
Gregory et al.(2008. 2010) have developed and tested an integrative
psychodynamic modelfor co-occurring borderline PD and alcohol
use disorders that has shown a promising acute treatment and follow-
up (Gregory et al., 2010) symptom improvement. An important
question is whetherthe increased time, cost, and complexity of
training, supervision, and delivery ofintegrative models for dual
disorders is justified based on their superior outcome compared with
existing evidence-based therapies focused mostly on the symptoms of
one ofthe disorders. To date,there is limited supportforthe use of
DBT in substance-dependent patients with borderline PD compared
with an addiction-focused comparison therapy (Linehan et al., 2002;
van den Bosch et al., 2002). A very small clinicaltrial suggested that
DBT may be betterforthis population than would be an uncontrolled
therapy-as-usual,for which limited symptom change seemed best
explained by poorretention and utilization (Linehan et al.,1999).
Similar concerns aboutthe small sample size and the differentialin-
tensity of an uncontrolled treatment-as-usual can be raised aboutthe
promising dynamic deconstructive psychotherapy model by Gregory
et al.(2008. 2010). The current study compared the efficacy of
the dual-focus schema tlierapy (DFST; Ball,1998. 2004; Ball and
Young, 2000), which treats a broaderrange ofPDs and substance de-
pendence, versus individual drug counseling (IDC; Mercer and Woody,
1999), which specifically targets addiction symptoms. Both DFST and
IDC were manual-guided weekly individualtherapies delivered during
the first 6 months in a residentialtherapeutic community.
We have focused on PDs and substance dependence because
these are the most common forms of co-occurring psychiatric dis-
orders and are especially prevalent(70% to 90%)in inpatient or
residential programs (Verheui et al.,1998a). Numerous studies have
found that PDs are associated with worse substance-related outcomes
when provided with routine or standard addiction treatment(Verheui
et al., 2005) butthatthe poor prognosis of antisocial and borderline
PD may be mitigated by adding psychotherapeutic (Cacciola et al.,
1996; Woody et al.,1985), psychiatric (Nace and Davis,1993),
contingency management(Messina et al., 2003), ortherapeutic
community interventions (McKendrick et al., 2007; Ravndal et al.,
2005). Forthis reason, we developed and tested DFST as the first
integrative cognitive behavioraltherapy forthe fullrange of PDs
found in substance-dependent patients (Ball,1998; 2004). DFST
showed promise in two smallrandomized trials when tested against
an individual12-step facilitation therapy in methadone-maintained
patients (Ball, 2007) and against group drug counseling with home-
less substance-abusing clients (Ball et al, 2005)
躁うつとうつ
双極性障害と境界性人格障害の鑑別と共存
まずは抄録を以下に引用。
「近年,境界性人格障害と双極性障害の鑑別が重要視される背景には,操作的診断に基づいた境界性人格障害の安易な診断と,境界性人格障害様症状を呈する双極性障害の増加がある。
とはいえ,境界性人格障害の診断基準に掲げられたほとんどの症状は感情障害でも観察されるために,両者の鑑別と共存が問題になる。
とりわけ,双極Ⅱ型障害のうつ病相や気分循環性気質などの微細な躁的因子を内包する病態で,境界性人格障害様の症状は出現しやすい。
この場合は,同一性の障害や特有の防衛機制の有無などに注意を払い,境界性人格障害と鑑別して気分安定薬中心の処方をする必要がある。
他方,境界性人格障害に双極性障害が合併するケースでは,境界性人格障害の人格病理を踏まえた精神療法的な対応に加え,同時に存在する双極性障害にも目配りし,薬物療法的な配慮を忘れてはならない」
双極性障害(躁うつ病,躁鬱病)の経過中に境界性人格障害を思わせるような症状が出現することがしばしばあるので診断には慎重にならなければならない。
一方で,双極性障害と境界性人格障害がそれぞれ独立した疾患として同じ患者さんに併存することもあるので,その場合は両者のバランスを考えた治療が必要になります。
以下に,この論文の本文をできるだけわかりやすく要約してみましょう。
Ⅲ.境界性人格障害の診断基準にみられる「感情障害」の要素
感情障害とはうつ病や双極性障害の総称です。
この項で筆者は,DSM-IV-TRの境界性人格障害の診断基準を取り上げ,感情障害との関連を論じています。
(1) 「現実に,または想像の中で見捨てられることを避けようとするなにふりかまわない努力」→軽うつ状態で依存性が目立つようになったうつ病症例でも認められる。
(2) 「『理想化』と『こきおろし』の両極端を揺れ動くことによって特徴づけられる不安定で激しい対人関係の様式」→軽躁状態ないし混合状態でも認められる。
(3) 「同一性障害」→躁とうつの転換が頻回な症例では,「本来の自分がわからない」と自己像が不安定になることがある。
(4) 「自己を傷つける可能性のある衝動性で,少なくとも2つの領域にわたるもの(例:浪費,性行為,物質乱用,無謀な運転,むちゃ食い)」→いずれも軽躁状態や双極Ⅱ型障害のうつ病相でよく認められる。
(5) 「自殺の行動,そぶり,おどし,または自傷行為の繰り返し」→もちろんうつ病でも認められる。
(6) 「顕著な気分反応性による感情不安定性」→混合状態でも認められる。
(7) 「慢性的な空虚感」→慢性経過をたどるうつ状態や気分変調性障害でも認められる。
(8) 「不適切で激しい怒り,または怒りの制御の困難」→軽躁状態,混合状態でも認められる。
(9) 「一過性のストレス関連性の妄想様観念,または重篤な解離症状」→うつ病相でも認められる。
(1)~(9)のうち5つが認められれば境界性人格障害と診断されることになっているが,ここに示したように,軽躁状態や混合状態を呈していればこの基準を満たしてしまうことは十分ありうる。
双極性障害と境界性人格障害の症状が同時に現れる場合は,双極性障害の診断が優先されるべきである。
Ⅳ-1.境界性人格障害と誤診される双極性障害
よく問題になるのは,手首自傷や大量服薬による頻回の自殺企図に及ぶ双極性障害の症例。
入院させてみるとむしろ軽躁的な面も目立ち,病棟内で逸脱行動を呈することも少なくない。
治療者の些細な言動に対し「見捨てられた」と思い込み,行動化に至る。
治療者はどうしても表面上の問題行動に目を奪われがちで,背景にある微細な気分変動を見逃すことになりやすい。
人格の問題として突き放したり,(境界性人格障害の治療に準じて)厳格な限界設定を試みて治療関係がさらに悪化することもしばしばである。
患者さん本人も躁やうつのために自分の本来の性格や生活史について正しい評価ができず,「本来の自分がわからない」といった(境界性人格障害の診断基準のひとつである)同一性の障害を疑わせる発言をする。
治療者に対する患者の態度は気分の変動に左右されてくるくる変わることがあり,「理想化をこきおろし」というこれまた境界性人格障害の診断基準を満たすことになる。
こうして,衝動制御の障害,感情不安定性,同一性の障害,対人関係の障害といった境界性人格障害の診断項目が揃ってしまう。
ここで機械的に境界性人格障害の診断を下し,告知をすると,患者が絶望し,本来の双極性障害の経過に悪影響を及ぼすことになりかねない。
しかし詳細に病歴を聴取してみると,幼少期の発達や人間関係は問題なく,学校や職場でもそれなりに評価されていることが少なくない。
こうした,家族や職場での評価が,(人格障害ではなく双極性障害の)診断を下す決め手となる。
このような病像を呈しやすいのは双極Ⅱ型障害である。
治療者は,うつ病や躁うつ病の症状の出現様式が境界性人格障害の臨床特徴と重なる部分が多いことを認識しておくべきである。
また薬物の選択も重要である。
双極性障害の患者を人格障害と誤診した場合,抑うつ症状や衝動性を人格障害の症状と解釈してSSRIを中心とした抗うつ薬が使用されるが,このことで混合状態や急速交代化が起こり,より問題が複雑化することがある。
Ⅳ-2.双極性障害と誤診される境界性人格障害
また治療者への陽性転移,これに引き続く陰性転移において,気分の高揚,または落ち込みといった気分変動が交互に出現することがある。
そうした場合,治療者は転移現象が意味していることについて全く理解せず,双極性障害と診断し,薬物療法一辺倒の治療を試みることがありうる。
このように,境界性人格障害における転移下の気分変動を真正の気分障害と誤診してしまうことにも十分な注意が必要であろう。
たしかに,この気分変動はDSM-IV-TRに照らせば気分障害と操作的に診断される可能性があるが,治療において両者は区別されるべきである。
おわりに
近年,双極性障害が境界性人格障害か,あるいは両者の合併かと議論されるケースが増えた印象がある。
臨床上は感情不安定性や衝動性の背後に双極性障害が潜在する可能性を絶えず念頭に置いて治療していく必要があろう。
ようするに,
(1) 双極性障害と境界性人格障害の鑑別は難しい。
(2) 双極性障害を境界性人格障害と誤診すると,精神療法に傾きがちで,必要な薬物療法を施すことができない。
(3) 境界性人格障害を双極性障害と誤診すると,薬物療法に傾きがちで,必要な精神療法を施すことができない。
